Unclaimed Property Program
Effective with the May 1, 2012 report for Life Insurance Companies and November 1, 2012 for all other Holders, the dormancy period for payroll will be 1 year.
Reporting Forms and Software
Unclaimed Property Reporting Forms may be printed below using either Microsoft Word (Microsoft Excel for ASD-127 [Abandoned Tangible Personal Property]) or Adobe Reader 5.0 or above. Once printed, using legal size 8.5 x 14 paper for all reporting forms except the ASD-127 which is standard 8.5 x 11, complete the form and mail it to the address listed on the form.
Holders reporting 50 or more property owner records are required to report electronically, either in the NAUPA Standard Electronic File Format (Excel files do not meet this requirement) or in the optional NC Electronic File Format (will be the required format beginning in 2012). For holders reporting less than 50 property owner records, the report may be filed electronically or in paper form. To meet this requirement, holders may utilize UPExchange (Help desk support for this software is available at (319) 739-3557) or HRS Pro which are free electronic reporting software. Property owner information may be manually entered into these reporting software or may be imported into the software from other electronic applications such as Excel.
For those holders reporting electronically, we are requesting that the ASD-159 (Unclaimed Property Verification and Checklist), or its equivalent, be submitted in paper format, along with a PDF file or paper copy of the owner detail to assist with the loading and balancing of holder reports.
Holder reporting tangible property should continue using paper reports ASD-127 (Abandoned Tangible Personal Property), regardless of the number of owners, unless advised otherwise.
NOTE: An ASD-159 or ASD-159G (Report of Unclaimed Property Verification and Checklist) must be completed, notarized, and submitted with any remitted report (cash, securities, or tangible filing). Forms not signed or notarized, if required, will not be processed and we be returned to the holder for correction. Any limiting notations or endorsements written or stamped on attached payment documents - to include any similar limiting language contained in any accompanying correspondence - cannot be reviewed and will not be honored.
Holders that do not have anything to report are not required to file a "negative" report. If you elect to file a negative report, an ASD-NEG (Negative Report of Unclaimed Property) must be completed.

Any property owing to owners with a last known address in California must be reported directly to California.

Holder Refund Request Form
Holders requesting a refund for funds submitted in error or refunded to owners should complete a Holder Refund Request Form. Once printed, using standard 8.5 x 11 paper, complete the form and mail it to the address listed on the form. Don't forget to include documentary evidence when submitting refund request. The request should be signed by two officers.
